How You’ll Make an Impact?

The Clinical Social Worker is responsible for the clinical oversight of all participants within the Family Intervention program, ensuring that every intervention is grounded in professional standards and therapeutic best practices. This role leads the development and rigorous monitoring of care planning and adherence, ensuring that service delivery remains aligned with established participant goals.

 

Specializing in complex case management, the Clinical Social Worker provides direct intervention and strategic coordination for high-needs cases, bridging the gap between clinical assessment and integrated community support.

  • Provide expert clinical guidance across the program to ensure all participant interventions are evidence-based, trauma-informed, and aligned with professional social work ethics.
  • Lead the development, implementation, and regular review of multi-disciplinary care plans, ensuring strict adherence to treatment goals and service standards.
  • Act as the primary lead for high-acuity cases, navigating complex psychosocial factors and coordinating intensive support for participants with multi-faceted needs.
  • Conduct ongoing clinical risk assessments and implement safety plans to proactively address crisis situations and ensure participant stability.
  • Facilitate communication between service providers, community partners, and internal teams to ensure seamless, wrap-around approach to participant care.

What You’ll Bring?

  • Master’s Degree in Social Work.
  • Active clinical registration with the Alberta College of Social Workers (ACSW).
  • 3-5 years of direct clinical experience, specifically performing psychosocial assessments and developing multi-disciplinary care plans.
  • Proven experience in a clinical setting, specifically in managing multi-disciplinary care plans and high-acuity caseloads.
  • Demonstrated ability to perform clinical psychosocial assessments and risk screenings.
  • Experience working with addictions, mental health, disabilities in the social service sector is considered an asset
